Paperwork for Medications at School

If your child needs an allergy action plan or medication administered at school, please click here and scroll down to expand the Medical Management section to find the appropriate forms. The prescription medication form must be completed by a physician so give yourself adequate time to get this form completed. Forms can be faxed to our office at 248.341.7199. Parents may call the office to arrange to drop off a child's medication, preferably on or before the first day students attend in-person.

All medications, both prescription and non-prescription, must be stored and administered in the office and turned into the office in their original containers.
